Additional information regarding you and your child’s participation in PABA is:

ALL PARTICIPANTS must sell twenty (25) Raffle tickets at $5.00 per ticket (total $125.00). These tickets must be prepaid along with the registration fee during the registration process. Each Raffle Ticket stub must be filled out completely by the buyer, with their full name, complete address and phone number.

You may pay here using any major credit card. If you would like to pay by check, cash or via your Independent Health benefits card, choose payment by check on the payment screen. Then you may bring payment to the first practice.
